PATIENT NEWS: SHARE FOR YOU:
A new way of working to support organisations involved in your care across Heywood, Middleton & Rochdale. This will ensure that professionals involved in your care have access to the information they need to provide you with the best care possible. We understand that it can be frustrating and difficult to repeat details of your symptoms and medical history each time you see a different doctor, nurse or social care professional, or when you visit a hospital or A&E department. Only professionals directly involved in your care will be able to see your personal information through ShareforYou. We will only access your ShareforYou record with your consent, unless this is not possible and access is required as part of your care e.g. you are unconscious, or there is a legal requirement to do so. If you would like to opt out, please contact the surgery.
What will be shared? Your recent diagnosis and test results, any allergies you have, medications and treatments you currently receive, any current or past (significant) illnesses, encounters and referrals.
How do I know my records are secure? By law, everyone working in, or for, the NHS and Adults' and Children's Social Care must respect your privacy and keep your information safe and confidential. Your information is stored on secure computer systems connected to a secure health & social care network.

When booking G.P. appointments online please note that G.P's can only deal with one problem in one ten minute appointment. Therefore, if you need to discuss more than one problem in one consultation, please ring reception and they will book you a longer appointment.
G.P. appointments are not for new patient checks or administration queries. New patient checks are done by the Practice Nurses/Healthcare Assistant. Due to the Nurses/Healthcare Assistants having many different appointments which take different amounts of time, you cannot book these online, you can ring through to reception who can book these for you on 01706 378476. Similarly, Administration queries can be dealt with via ringing reception staff.
